CareStack
Where do I send him, and will they actually take him?
Every residential program runs on the same three things: a binder, a spreadsheet, and one veteran staff member who knows which places pick up the phone. All three fail the same way.
CareStack is a resource directory that stays true, and a referral tracker that keeps every open thread visible. Built for care coordinators, by one.
What it does
Tells you who a place will turn away
Most directories list who qualifies and never who doesn’t, so coordinators discover exclusions on the phone — or a man discovers them at the door. Every entry names its exclusions.
Says “not published — ask” instead of guessing
Every fact is checked against a primary source and dated. Where something isn’t published — a medication policy, a record policy, a price — the entry says so rather than inventing an answer.
Gives you the words to use on the call
Ask for the Rapid Reentry track by name. Say plainly that you can’t pay — nobody is denied for that. The thing that isn’t written anywhere, written down.
Won’t let a referral move on a guess
A referral can’t advance past the call until the medication and record policies are recorded. The refusal states the question to ask.
Keeps what your team learns
“Ask for Denise on weekday mornings, she’ll hold a bed.” The institutional knowledge that currently lives in one person’s head and walks out when they leave.
Where it stands
112 verified services across 19 categories, covering Greater Boston — a bed tonight, detox, housing, work, documents, benefits, record sealing, legal help, healthcare, medication continuity, recovery community, money, education, and the things that are invisible until they stop someone: food, clothes, a phone, the fare.
In pilot at a men’s recovery residence on Massachusetts Avenue, Boston.
The application is in private pilot. Access is by invitation from a program administrator.
CareStack is a coordination tool, not a clinical record, and not a substitute for a person walking someone through a door.
It holds no medical information.
